Privacy Policy

Peak Swing Physical Therapy (“Peak Swing,” “we,” “us”) is a physical therapy practice at 7640 South Tamiami Trail, Sarasota, FL 34231. This policy explains what information we collect through peakswing.com and the ads that lead to it, how we use it, and the choices you have.

This policy covers the website. Your clinical records, once you become a patient, are kept separately and handled under our patient privacy practices, which we're happy to share on request.

When you use a form on this site we collect what you enter, such as:

• Booking an evaluation:your name, email, phone number, the service you're interested in, the date and time you choose, and any notes you add.
• Contact form and phone-consult requests: your name, email and/or phone number, a good time to call, and your message.
• Guides, newsletter and waitlist sign-ups: your name and email (and phone, for the waitlist).
• Workshop waivers: the details the waiver asks for.

Please don't include detailed medical history in a website form. We'll go over that with you in person or by phone.

Like most websites, we and our partners use cookies, pixels and similar technologies to understand how the site is used and how well our advertising works. These may collect:

• pages you view, links and buttons you click (for example, tapping our phone number), and when;
• your device, browser, approximate location from your IP address, and the website or ad that sent you here (including campaign tags in the link, such as “utm” or click identifiers).

The tools we use are:

• Google(Google Analytics, Google Ads and Google Tag Manager), including Google's call tracking, which may show a Google forwarding number on our site when you arrive from a Google ad so we can tell whether the ad led to a call.
• Meta (the Meta Pixel and Conversions API), which tells Facebook and Instagram when someone views a page, taps to call, or sends us a form, so we can measure and improve our ads.

When you send a form, we may share your email address and phone number with Google and Meta in a hashed (scrambled) form so they can match the action to an ad. We do not send them the service or condition you choose when booking, or the contents of your message.

• to respond to you, schedule and confirm appointments, and return calls;
• to send the guides or newsletter you asked for (you can unsubscribe at any time);
• to ask about your experience after a visit, including requests for reviews;
• to understand which pages, ads and campaigns bring people to us, and to improve them;
• to keep the site secure and prevent spam and abuse;
• to meet our legal and professional obligations.

We do not sell your personal information. We share it only with service providers that help us run the practice, who may use it only to provide their service to us:

• website hosting and our website database (Netlify, Neon);
• email delivery (Resend) and our scheduling calendar and records (Google Workspace);
• our client-relationship and phone-answering tools, which also answer and log calls to our number;
• advertising and analytics partners (Google and Meta), as described above.

We may also disclose information if required by law, or to protect the rights, safety or property of our patients, our practice or others.

• You can block or delete cookies in your browser settings. The site still works without them, though some features (like the booking form) may need to reload.
• You can limit ad personalization at Google's ad settings and in Meta's ad preferences, and opt out of Google Analytics with Google's browser add-on.
• Every email we send has an unsubscribe link.

To ask what information we hold about you, to correct it, or to have it deleted, email jay.thornton@peakswing.com or call (941) 203-5305. Tell us the name, email and phone number you used. We'll confirm it's you and respond within 30 days. Deleting your website information doesn't delete clinical records we're required by law to keep.

We keep form submissions and sign-ups for as long as we need them to serve you or as the law requires, and delete them when they are no longer needed. We use reasonable safeguards, including encrypted connections and access limited to the people who need it, but no website or service is perfectly secure.

This site is meant for adults. We don't knowingly collect information online from children under 13. Parents scheduling care for a child should call us.
